Terms of Service

Welcome to H-MD. By scheduling an appointment or purchasing our services, you agree to the following terms and conditions.

Cancellation and No-Show Policy Appointments must be cancelled or rescheduled at least 48 hours in advance. This notice allows us to offer the time slot to other patients.

  • Late Cancellation: Cancellations made within 48 hours of the scheduled appointment time will incur a $50 fee.
  • No-Show: Failure to attend a scheduled appointment without prior notice will incur a $100 fee.
  • One-Time Courtesy: At the practice’s discretion, we may waive the cancellation or no-show fee once as a courtesy. This waiver will be documented in the patient’s record. After this first occurrence, all subsequent late cancellations or no-shows will result in the applicable fee.
  • Payment Method: All late cancellation and no-show fees will be charged to the card on file. We may require a valid credit card to be kept on file prior to scheduling future appointments.

Gift Card, Promotion, and Discount Policy The following terms apply to all gift cards, promotions, and discounted offers:

  • All Sales Are Final: This applies to all gift card and promotional purchases.
  • Restrictions: Cannot be combined with other offers. No substitutions are permitted.
  • Value: Gift cards and promotional offers have no cash value and are non-transferable.

Product Sales and Returns

  • Unopened Products: May be exchanged or returned for a full refund within 30 days of purchase. Refunds or exchanges may be subject to a 5% restocking fee.
  • Opened Products: There are no refunds on opened products for health safety reasons. However, we may offer store credit at our discretion.
  • Adverse Reactions: If an adverse skin reaction occurs, we may offer a full refund in the form of store credit.

Discounted Packages and Services H-MD offers the ability to pay per treatment (pay-as-you-go) or receive a discount for purchasing a package upfront.

  • Package Disruption: If a package is disrupted prior to completion, the discount is forfeited and you will be charged the pay-as-you-go list price for completed treatments.
  • Refund Requests: Any cancellation or refund request for a package must be submitted in writing.
  • Calculations: If you discontinue a package before completion, you will be charged the list price of the completed procedures plus a 10% service charge. Any resulting refund will be paid 30 days after the written request is received.
  • Example: A single treatment is $125, but a package of 6 is $625. If a refund is requested after 3 treatments, the charge is ($125 x 3) + 10% service fee = $412.50. The refund amount would be $212.50.

Age Requirements

  • Dermal Fillers: Must be 21 years of age or older.
  • Neurotoxins (Tox): Must be 18 years of age or older.
  • All Other Treatments: Must be 18 years of age or older unless otherwise approved by clinical staff with parental consent.

Policy Changes H-MD reserves the right to update or modify these policies at any time. The current policy will be maintained at the clinic and on our website.
